Incorrect GRACE score
If you enter 69y, HR 110bpm, SBP 120, krea 2, Killip 4 and yes to all the questions you will get very optimistic 1.9% probability of death (the correct value should be about 70%)

I believe the issue is that your numbers are based on the original Granger paper and should be based on the nomogram referenced in the GRACE collaborators calculator methods paper ( www.outcomes-umassmed.org/grace/publicfiles/GRACE_RiskModel_Coefficients.pdf )

I get a risk of 0.0%, not the 1.9%. But yes, there is an issue for scores > 250, in which case the result should be >= 52%, not 0%.
