Correction on the serum osmolality formula information
Glucose should be divided by 18. So it should be (2 x Na) + (glucose/18) + (BUN / 2.8) source: Kaplan 2003, p.41

This would be necessary if glucose was in mg/dL and you’d need to convert it to mmol/L. However the formula as given states that glucose already is in mmol/L.
