Opioid equivalency calc needs newer opiates added
The opiate conversion calc is great, except that it is missing some newer opiate medications. The main one that is missing is Oxymorphone, also known as Opana or OpanaER. This drug is becoming as big as oxycodone for long term care of chronic pain and cancer patients due to its potency, so including conversion formulas in the opiate equivalency calc would be a wise decision and benefit a lot of people, especially since there is a lot of conflicting data out there, sometimes saying that its equivalence to oxycodone is 1:2 respectively and other times ill see its equivalence stated as 1:1.5. So adding Oxymorphone to your calc and including its true equivalence would be a huge benefit to doctors and patients alike. Thank you

you can add a conversion level analgesic paracetamol 1 g X 4 in analgesic level II ou III
And add a conversion betwin morphine per os to fentanyl trans dermique

This is exactly the problem: sometimes it's stated as 1:2 and sometimes 1:1.5 – we do not want to be the authoritative source for which factor to use. And no matter which conversion we implement, there will be complaints. So we are very hesitant in implementing conversions where there are different factors circulating around.
