Balthazar CT severity index
I think you should add the Balthazar CT severity index for pancreatitis. I am a Surgical ICU director, and I would use this all the time.
Balthazar EJ. Acute pancreatitis: value of CT in establishing prognosis.
Radiology 1990; 174:331-6.
Step 1:
Amount of pancreatic edema is estimated
A normal
B diffuse enlargement of the pancreas without peripancreatic inflammatory changes
C enlarged pancreas with haziness and increased density of peripancreatic fat
D enlarged body and tail of pancreas with fluid collection in peripancreatic spaces
E extensive fluid collections in lesser sac and anterior pararenal space
Then each grade is given a score
A=0, B=1, C=2, D=3, E=4.
Step 2:
Then an estimation of pancreatic necrosis is made:
0=0 points
1 to 30%=2 points
31 to 50%= 4 points
51-100% = 6 points
Step 3:
Then the points from step 1 and 2 are added up to get a total point score.
This is the Balthazar CT severity index for acute pancreatitis.
0-3: Mild pancreatitis
4-6: moderate pancreatitis
7-10: severe pancreatitis
This score can be very important for ICU triage.
