There are many good references to safe local anesthetic dosing and max dosing.
As Anesthesiologists we are comfortable dosing local anesthetics.
I don't have a problem with a recommendation table that is accompanied by the usual warnings.

This is on the barash hand book:
Max Doses
Lidocaine 4.5mg/kg (300mg max)
Lidocaine + Epi 7mg/kg (500mg max)
Bupivacaine 2mg/kg (175mg max)
Bupivacaine + Epi 3mg/kg (225mgmax)

This would be a tough one, because local anesthetic max doses are controversial, no one really knows what they are, and it depends so much on WHERE you put it. See "Maximum recommended doses of local anesthetics: a multifactorial concept": http://www.ncbi.nlm.nih.gov/pubmed/15635516

As mentioned in a comment to another suggestion (https://medcalc.uservoice.com/forums/104899-formula-suggestions/suggestions/2438018-maximum-allowable-local-anesthetic-doses), I'm not sure this can be done right without posing patients in danger. If somebody has a good reference demonstrating the safety of such a table, please add it here.

Not a very good idea! There actually aren't any! It depends too much on the exact spot of injection, the perfusion of this area, the patient's health status, other drugs etc.
