FINDRISC or CANRISK
Finnish well validated screening score for diabetes risk
Canadian screening score/calculator for diabetes risk
From the Canadian Task Force on Preventative Med:
FINDRISC (the Finnish Diabetes Risk Score) has been selected as the preferred validated risk calculator, but CANRISK (the Canadian Diabetes Risk Assessment Questionnaire) is an acceptable alternative. Factors considered in FINDRISC and CANRISK are age, obesity, history of elevated glucose levels, history of hypertension, family history of diabetes, limited activity levels, and diet with limited intake of fruits and vegetables. The CANRISK questionnaire can be found via the PHAC website or here:

DECAF score
The DECAF score: predicting hospital mortality in exacerbations of chronic obstructive pulmonary disease
Steer J et al
These researchers analysed admission data and in-hospital deaths among 920 consecutive patients hospitalised for a COPD exacerbation to identify outcome predictors; mean FEV1 was 43.6% of predicted and 10.4% of patients died in hospital. They then combined the five strongest mortality predictors to form the ‘DECAF’ score. DECAF exhibited excellent discrimination for mortality (area under the receiver operator characteristic curve 0.86 [95% CI 0.82, 0.89]). Moreover, DECAF performed better than other clinical prediction tools; for example, it was a significantly stronger predictor of…

Mayo Score for Assessment of Ulcerative Colitis Activity
http://www.globalrph.com/mayo_clinic_score.htm
Accurate assessment of disease severity and degree of active inflammation are essential to optimizing treatment. No single, validated disease index presently exists for UC. Although previously we relied on clinical criteria to assess disease severity, a more recent tool -- the Mayo score -- combines clinical and endoscopic subscores.
