1. Riesgo de caídas

   Las caídas representan la segunda causa de muerte en todo el mundo por lesiones accidentales o no intencionales. Se calcula que anualmente se producen 37,3 millones de caídas que necesitarán atención médica, resultando mortales en el 8,8% de los casos.

   Las causas de las caídas pueden ser muy diversas y estar relacionadas con:

   Alteraciones del equilibrio.
   Problemas en la marcha.
   Pérdida de fuerza muscular.
   Trastornos cognitivos.
   Alteraciones de la visión.
   Padecer determinadas enfermedades crónicas.
   Seguir un tratamiento con psicótropos.

   O bien a aspectos relacionados con las condiciones del entorno, como:

   Suelos resbaladizos.
   Iluminación insuficiente.
   Superficies irregulares.
   Calzado y ropa inapropiados.
   No disponer de ayudas para caminar y desplazarse.
   Barreras arquitectónicas, etc.

   Existen diferentes escalas que permiten evaluar el riesgo de caídas de una persona atendiendo a distintos criterios. Este es el caso de la escala de Downton, que se utiliza para evaluar el riesgo que presenta una persona de sufrir una caída con el fin de, en caso necesario, poder implementar medidas preventivas encaminadas a evitar nuevas caídas.

   Esta escala tiene en cuenta si ha habido o no caídas previas, las causas que la han producido o los factores de riesgo que presenta en este sentido cada paciente y la edad, asignando a cada punto un valor de 0 ó 1. Cuando el resultado de la escala es de tres o más puntos se considera que el paciente tiene un alto riesgo de sufrir una caída.

   Entre los factores que se analizan figuran los siguientes:

   Caídas previas: Sí (1).
   - No (0)
   Medicación:
   - Ninguna (0).
   - Tranquilizantes o sedantes (1).
   - Diuréticos (1).
   - Hipotensores no diuréticos (1).
   - Antiparkinsonianos (1).
   - Antidepresivos (1).
   - Otros medicamentos (1).
   Déficit sensoriales:Ninguno (0).
   - Alteraciones de la vista (1).
   - Alteraciones auditivas (1).
   - Alteraciones de las extremidades causadas, por ejemplo, por haber sufrido un ictus, una enfermedad neurológica, etc. (1).
   Estado mental:
   - Orientado (0).
   - Confuso (1).
   Deambulación:
   - Normal (0).
   - Segura con ayuda (1).
   - Insegura con o sin ayuda (1).
   - Imposible (1)

   Una vez realizada la evaluación del riesgo de caídas mediante la escala de Downton existen protocolos de actuación que contemplan la asignación del paciente a un grupo de bajo o alto riesgo y la adopción de medidas preventivas básicas en el primer caso y de otras específicas en el segundo, en función de los factores de riesgo que presente. Además, se realiza también la educación del propio paciente, de sus familiares o de la persona que actúa como cuidadora para que asegure el seguimiento de estas medidas.

   También se deben notificar las caídas que se puedan producir y se debe proceder a la reevaluación del paciente en el caso de que se haya producido una caída o de se produzcan cambios clínicos relacionados con su enfermedad.
