  5. FINDRISC or CANRISK

Finnish well validated screening score for diabetes risk
Canadian screening score/calculator for diabetes risk
From the Canadian Task Force on Preventative Med:
FINDRISC (the Finnish Diabetes Risk Score) has been selected as the preferred validated risk calculator, but CANRISK (the Canadian Diabetes Risk Assessment Questionnaire) is an acceptable alternative. Factors considered in FINDRISC and CANRISK are age, obesity, history of elevated glucose levels, history of hypertension, family history of diabetes, limited activity levels, and diet with limited intake of fruits and vegetables. The CANRISK questionnaire can be found via the PHAC website or here:

  8. GAP prognostic index for IPF

This article has a calculator that allows physicians to calculate expected prognosis in IPF. Generally clinicians quote a median survival of 3 years for all of their patients at the time of diagnosis. This paper clearly allows us to stratify patients into 3 groups with very different prognoses. However the calculation is somewhat cumbersome and without a "medcalc" option, clinicians are likely to avoid doing the calculation and therefore prevent patients with this devastating disease from getting a more accurate prognosis.

Reference: Ley et al. A multidimensional index and staging system for idiopathic pulmonary fibrosis. Ann Intern Med 2012; 156:684-691.
